Suggestions & Complaints

The registration of a healthcare institution is done at the Department of public health located at the vineyard building. The form must first be filled in and should include the following documents:

  • Articles of incorporation
  • Annual financial statements, approved by a Certified Public Accountant (CPA) or a Registered Accountant (RA), if the application pertains to an already existing health care institution
  • Operating budget
  • Copy of payment for processing fee

The documents can then be submitted to the Department of Public health.
